Confirm your Ride

Please fill out this credit card authorization form- your reservation is not confirmed until we receive it. Please note that submitting this form does not pay for your ride. It is simply to HOLD YOUR RESERVATION and you will still need to pay your driver by cash or card. If you would like to keep your credit card on file and use as a form of payment, please call the office and speak to Mike. You must fill out this form regardless if you are paying by cash or credit card.

Cancellations: Please understand we require a 24 hour notice for cancellations for pickup at any airport. If we do not receive verbal or written cancellation we will charge the credit card for the full price of the ride.
